Bridging the Gap Between SNF and Home- Part 2

This course explores geriatrics and the rehabilitation setting and what makes rehabilitation unique with older adults. We explore bridging the Gap between the rehabilitation setting and discharge to home to help set up patients for the highest rate of success. We explore Active aging; as well as physical, cognitive and leisure education interventions that can be used at the facility as well as at home to give patients the tools and knowledge to maintain an active aging lifestyle.

Course Objectives

  • Describe 2 personal and 2 environmental factors that may influence the rehabilitation experience

  • Explain 2 ways that Recreational therapy can benefits older adults in the rehabilitation setting

  • Define Pre-discharge counseling and explain why it is important

Angela Goldschmidt, CTRS

Instructor Bio:

Angela Goldschmidt is a certified therapeutic recreation specialist (CTRS) through the National Council for Therapeutic Recreation Certification (NCTRC). She has a Bachelors of Science, Recreational Therapy with minors in Gerontology and Socio-Psychology from Mount St. Joseph University in Cincinnati, OH. She has 12 years CTRS experience in the Long Term Care, Geriatrics and Physical Rehab settings and over 22 years experience with the geriatric population. Currently a stay-at-home mom to 4 girls, she is excited to be able to share her experience and knowledge of therapeutic recreation for the geriatric population.
